Legal Policies and Disclaimers
This page explains the policies, notices, and disclaimers that apply when using the Virtual Wellness Care, LLC website or receiving telehealth services. Please review each section carefully before submitting information, booking an appointment, or beginning care.
Because these are healthcare and legal policies, the final wording should be reviewed by a qualified healthcare attorney or compliance professional before publication.
Virtual Wellness Care may collect information that users voluntarily provide through the website, including information submitted through contact forms, appointment requests, registration forms, or other website features. This information may be used to respond to questions, support appointment scheduling, provide requested services, and manage communication with patients or prospective patients.
Virtual Wellness Care takes reasonable steps to protect information submitted through the website. However, users should avoid sending detailed medical information, highly sensitive personal information, or urgent medical concerns through standard website forms or email. Information related to patient care should be shared through approved secure systems when available.
The HIPAA Notice of Privacy Practices explains how protected health information may be used and disclosed by Virtual Wellness Care and how that information may be protected in accordance with applicable HIPAA requirements.
The notice also explains patients’ rights regarding their health information, including the right to request access, corrections, restrictions, or information about certain disclosures where applicable. Patients should review the full Notice of Privacy Practices before receiving care.
Virtual Wellness Care provides healthcare services through telehealth, which may include secure video visits or secure patient portal messaging, depending on the service and clinical need. Telehealth allows patients to receive care remotely without attending an in-person appointment.
By receiving telehealth services, patients acknowledge that virtual care has benefits, risks, limitations, and privacy considerations. Some conditions may not be appropriate for telehealth and may require laboratory testing, diagnostic imaging, an in-person examination, referral, urgent care, or emergency evaluation.
Virtual Wellness Care does not provide emergency medical or crisis services. Website forms, email, voicemail, telehealth appointments, and patient portal messaging should not be used for urgent or emergency concerns.
Patients experiencing chest pain, severe shortness of breath, stroke symptoms, a severe allergic reaction, suicidal thoughts, or any other medical emergency should call 911 or go to the nearest emergency room immediately.
Information provided on the Virtual Wellness Care website is intended for general educational and informational purposes only. It is not a substitute for professional medical advice, diagnosis, treatment, or an evaluation by a qualified healthcare provider.
Using the website, reading its content, or submitting a general inquiry does not establish a provider-patient relationship. A provider-patient relationship is established only after the required intake process, consent forms, and appropriate clinical evaluation have been completed.
Not every medical or mental health concern can be safely evaluated, diagnosed, or treated through telehealth. The provider will determine whether virtual care is appropriate based on the patient’s symptoms, medical history, available information, and clinical needs.
Some concerns may require laboratory testing, diagnostic imaging, an in-person examination, urgent care, emergency care, specialist referral, or follow-up with a primary care provider. Telehealth treatment, medication, or a specific outcome cannot be guaranteed before an evaluation.
Virtual Wellness Care does not replace the importance of having an established primary care provider. Telehealth services are intended to support eligible health needs but are not a substitute for routine, preventive, and comprehensive primary care.
Patients are encouraged to maintain regular care with their primary care provider for physical examinations, preventive screenings, vaccinations, chronic condition management, and other ongoing healthcare needs.
Medications are prescribed only after an appropriate clinical evaluation and only when the provider determines that the medication is clinically appropriate. A telehealth appointment does not guarantee that a prescription or medication refill will be provided.
Prescriptions, refills, antibiotics, controlled substances, and weight loss medications may require additional medical information, laboratory testing, follow-up care, or an in-person evaluation. The provider may decline to prescribe a medication when it is not considered clinically appropriate or safe.
Controlled substances may be limited or unavailable through Virtual Wellness Care. These medications are prescribed only when clinically appropriate and when permitted under applicable laws, regulations, licensing requirements, and professional standards.
A request for a controlled substance does not guarantee that it will be prescribed. The provider may recommend alternative treatment, additional evaluation, referral, or in-person care when appropriate.
Medical weight loss treatment is individualized according to each patient’s medical history, health goals, clinical needs, medication options, medication dose, laboratory requirements, and safety considerations.
Results are not guaranteed, and weight loss medications may not be appropriate for every patient. Treatment recommendations may change based on clinical response, side effects, laboratory results, medication availability, or other health considerations.
Virtual Wellness Care accepts self-pay and select insurance plans. Insurance participation and coverage may vary by plan, service, state, network status, and individual benefits.
Patients are responsible for contacting Virtual Wellness Care and their insurance company before receiving care to verify eligibility, network participation, benefits, copayments, deductibles, prior authorization requirements, and non-covered services. Insurance verification does not guarantee payment by the insurance company.
A credit card is required at the time of booking to secure the appointment. The card will not be charged until after the visit when payment is necessary.
The final amount charged may depend on the type of service, self-pay status, insurance coverage, copayments, deductibles, non-covered services, or any remaining balance not paid by the insurance company. Patients remain responsible for charges that are not covered or paid by insurance.
Patients should provide at least 12 hours’ notice when cancelling or rescheduling an appointment. Cancellations made with less than 12 hours’ notice may result in a $25 cancellation fee.
Patients who miss an appointment without providing notice may also be charged a $25 no-show fee. Insurance plans may not cover cancellation or no-show fees, and the patient may be responsible for paying the fee directly.
Patients are responsible for providing accurate, complete, and current health information. This includes medical history, medications, medication allergies, symptoms, prior treatment, and any changes in health or condition.
Patients should also follow instructions provided by the healthcare provider and report new or worsening symptoms. Incomplete or inaccurate information may affect the provider’s ability to make appropriate and safe care recommendations.
Secure patient portal messaging may be used for routine, non-emergency communication between patients and Virtual Wellness Care. It may be used for appropriate questions, follow-up communication, appointment information, or other non-urgent matters.
Patient portal messages may not be reviewed immediately and should not be used for urgent symptoms, emergencies, or crisis concerns. Patients with urgent or emergency symptoms should call 911, visit the nearest emergency room, or seek appropriate in-person care.
